Form 3 is where most first article inspections actually fail — not
because the part is wrong, but because someone tried to summarize
instead of individually verifying every single characteristic called
out on the drawing.
What AS9102 actually requires
AS9102 is the aerospace standard governing First Article Inspection
(FAI) — a full, ground-up verification that a part, made using a
representative production process, actually conforms to every
requirement on its engineering drawing before that process is approved
for ongoing production. It’s mandatory in aerospace and aviation, and
the same discipline shows up in automotive, medical device, and
broader defense contracting as a production-validation gate.

Why Form 3 causes the most rejections
Form 3 requires every characteristic on the drawing — every
dimension, tolerance, note, and material callout — to be individually
numbered, measured, and recorded, cross-referenced back to the specific
balloon number on the drawing. The common mistake is treating this as a
summary exercise: skipping “obvious” or repeated characteristics,
grouping similar dimensions together, or leaving out notes that don’t
look like traditional dimensions. A customer reviewing a FAIR package
checks for that exact one-to-one completeness, and an incomplete Form 3
is the single most common reason a first article inspection gets
rejected and sent back.
When a new FAI is required
A completed FAI isn’t permanent. A change in the design, a change in
the manufacturing process or its location, a break in production, or a
change of raw material source can all trigger a requirement for a new
or partial first article inspection — the original FAI only certifies
the specific combination of design, process, and location it was
performed under, not the part number in the abstract.
