A customer rejecting a shipment doesn’t want a
five-minute explanation over the phone — they want a document, in a
specific format, walking through exactly what went wrong and exactly
what’s being done about it. In automotive and aerospace supply chains,
that document is almost always an 8D report.

Ford Motor Company formalized the Eight Disciplines methodology in
the 1980s as “Team Oriented Problem Solving,” originally to give
suppliers a standardized way to investigate and document quality
complaints. It spread through the automotive supply base and beyond
because it does something simple well: it forces a consistent structure
onto what used to be an inconsistent, ad hoc response to a customer
complaint.

Interim containment is the discipline people skip

D3’s interim containment action exists because root cause analysis
takes time, and a customer can’t wait for a finished investigation
before the bleeding stops. Containment might mean 100% sorting of
existing inventory, adding an inspection step, or temporarily changing
a process parameter — a stopgap, clearly labeled as temporary, that
protects the customer while D4 through D6 work out what actually
happened and how to fix it permanently.

supply chains, while A3 is generally an internal Toyota-style
single-page problem-solving format not necessarily meant to leave the
building.
