The atmospheric testing sequence in a confined space isn’t
arbitrary — oxygen first, then combustibles, then toxics, in that
specific order, using a calibrated direct-reading instrument. Testing
out of sequence, or skipping straight to a toxic-gas reading, can miss
a hazard the earlier tests were designed to catch first.

What makes a space “permit-required”

OSHA’s permit-required confined space standard, 29 CFR 1910.146,
applies to spaces large enough for a worker to enter, with limited
entry or exit, that aren’t designed for continuous occupancy — tanks,
vessels, silos, pits, and similar spaces — when they also contain or
have the potential to contain a hazardous atmosphere, engulfment risk,
an internal configuration that could trap a worker, or any other
serious recognized hazard. A space that meets the first, more general
definition but has none of those additional hazards is a “non-permit”
confined space and doesn’t require the full permit process.

Testing has to happen in the mandated sequence — oxygen, then
combustibles, then toxics — using a calibrated direct-reading
instrument, before anyone enters.

The three roles

The standard defines three distinct roles for every permit-required
entry: the authorized entrant, who does the actual work inside; the
attendant, who stays outside the space continuously, monitors
conditions, and can order an evacuation, with no other duties allowed
to distract from that job; and the entry supervisor, who verifies all
required testing is complete and rescue capability is in place before
authorizing entry, and who has the authority to cancel the permit if
conditions change. One person filling two of these roles at once
defeats the purpose of having them separated in the first place.

Why the attendant’s job is harder than it sounds

The standard explicitly prohibits an attendant from taking on other
duties that would distract from continuous monitoring — no running
parts to another area, no answering a phone that pulls attention away.
Rescue teams practicing simulated rescues at least annually, and
canceled permits retained for a year, both exist for the same reason:
a confined space rescue plan that’s never been practiced tends to fail
exactly when it’s actually needed.
